In keeping with current guidance and to reduce the risk to patients and staff, if a patient is attending the surgery can you please wear your own face mask or face covering, thank you.
******Due to latest Coronavirus Covid-19 advice we are changing the way we are working******
We are no longer booking routine appointments for the foreseeable future and the daily walk-in emergency clinic has been changed.
If you feel you need to see a doctor or a nurse please do NOT come to the surgery until you have spoken to a doctor or nurse over the phone and they have agreed that it is safe and sensible to do so.
Thank you for your understanding and cooperation in this difficult time.
From Thursday 19.03.2020 we shall be exclusively working from Yorkley Health Centre. This is for GP/nurse appointments and Dispensary.
******Please note that below advice/guidance is postponed until further notice due to the Coronavirus COVID-19 outbreak******
In both Yorkley and Bream we operate an appointment system for all consultations. You can make an appointment at either surgery.
Please contact the surgery between 08:30 – 13:00 and 14:30 – 17:00 to make an appointment. Some extra early morning and evening doctor’s surgeries will be available for those who find it difficult to attend the surgery during the day. In addition, in collaboration with other Forest Practices, weekday evening and weekend daytime appointments are now available either at this surgery, or at another surgery in the Forest. These appointments are bookable through this practice.
Emergency surgeries are held daily for urgent problems that cannot wait until the next available routine appointment. The doctor is able to spend more time with you in a routine appointment so please telephone the surgery to ask if there are any available before asking the time and place of the emergency surgery. Urgent emergency consultations are held at 11.00 on Monday and at 11:00 and 17:15 Tuesday - Friday on a first come / first seen basis for medical emergencies that cannot wait until the next routine appointment. Some book-on-the day emergency appointments are also available on Monday mornings for patients that need to be seen urgently.
If you would like to register for Online Services to enable you to make routine appointments via our website to see a doctor, for a blood test or to order repeat prescriptions, please call in to reception with proof of your identity.
If you are a new patient registering with the practice you will be offered a new patient check with the Practice Nurse. We do ask you to attend as this will enable us to gain information about your health and any medication that you may already be taking.
08:00 – 18:30 weekdays ring 01594 562 437 Option 1.
By Appointment Only
How Can You Help Us
Emergency surgeries held for conditions that cannot wait on medical grounds. Please do not use them just for convenience as this disadvantages those with a real need. Telephone to find out when and where the emergency surgeries are before attending. Please note that there is no emergency surgery at Bream on Mondays.
Please notify the receptionist of any change of name and/or address and telephone numbers. It is also important to inform the hospital if you are undergoing any treatment or waiting for test results or appointments, as your details will not automatically be updated anywhere else.
New babies should be registered with a doctor once they leave the hospital.
Please remember that an appointment is for one person only – if you need to consult about another member of your family, please make two appointments.
If you cannot attend an appointment for any reason, please inform us as soon as possible in order for us to give the slot to someone else.
